Notes field added to appointment #11

I have added the notes field to appointment entity class, created an EF migration, added it to the DTO, mapped it from DTOs to entity class and frontend.

Hi Hasan. Thank you again for contributing. The changes look good, but unfortunately I will not merge them because of legal reasons. I'm planning to maybe commercialize this product one day, and if you are in the list of contributors then there will be complications about code rights. I never planned to receive help from other people, so I'll remove the "good first issue" tags from my issues.

I'm wishing you luck with your future software development career and good luck with finding other open source projects that you can contribute to.
