Right now we are reading the contributor name from the emails that are sent out. If an author other than the corresponding one submitted the data after changing their name on the form, we are still giving credit to the corresponding author. We need to store the real contributor name. Note that there may be more than one contributor over time. We need to store multiple entries for the same paper. The feedback form should be also modified to 'remember' the last contributor when they come back.
