We probably want to store user information in a separate table, so we can secure it /delete easily, especially as their name/email isn't necessary for the stored template to be included in the main db.
Also, in addition to the above contents it would be good to ask for urls for the author/more information fields too. If possible we want to populate all of the email template fields from this form.
