If accepted, this PR will:

• Refactor all extractors to use a single entry point (a config trait)
• Refactor all transformers to use a single entry point (a config trait)
• Update PioglitazoneConfig to add a Config definition for every extractor and transformer used by the study. Some are hardcoded (e.g. Acts, FollowUp), others (e.g. Exposures) contain overridable config items that are read from the config file.
• Update PioglitazoneMain
