UI Update and minor fixes

• Update UI of the application
• Update Template to contain all the description for criteria
• Bootstrap popup used instead of default dialog boxes
• Show scores during an assessment
• Default date in Presentation date is current date;

@__@ I'm going to fix the Assessment Result popup to contain the scores for the criteria after I wake up.
