Misc updates for the XLS-70 spec

This PR:

Changes credentials to live in both the Subject and Issuer's owner directories, instead of just the reserve payer's. This makes lookup easier.
Removes the single-account creation flow (where the Issuer account doesn't have to exist on-chain).
- This was done to simplify the spec and because there was no known usecase for it. It can be added in a future amendment if there is a need.
Clarifies that the credential should be deleted if the Subject or Issuer accounts are deleted.
Clarifies additional failure cases.
Clarifies that the CredentialIDs field silently de-dupes.
Clarifies that credential-based Deposit Auth won't support the edge case for accounts that have a balance of less than the base reserve.

It also reorganizes the text so the changes to the DepositPreauth ledger object and transaction come after the credential transactions.
