Fix issue template #315

Context: Github issue templates are used on this repo to report bugs and request new features. This templates are in beta version and constantly change the API.

Description of the Change: The latest version of the template requires the label field to be present on checkboxes

Benefits: Fixes broken bug report and feature request forms.
