We could also try to visualize what happens if patient can also individually skip survey question cards.
Which questions do you want for the survey question cards before the result pages shows up?
Some of these questions look useful when asked after a recent visit.
Were you able to be seen by a doctor?
About How long did it take you to get set seen by a doctor after registration?
About How long did it take you to register for consultation after arriving at the healthcare facility?
About how long did it take you to to reach the healthcare facility?
The user isnt going to remember the exact address of where they went to visit but like you said maybe the city they remember most.
All the survey questions on the document have an asterisk the red looking symbol which means it is required for the user to answer. If the survey questions are going to be optional then we need the skip component.
Design buttons with appropriate style and text for each question card. Final question card may need to let users know they are finished and ready to view the search results.
