Implement a react based survey form

This is a single page work on an existing site. Implement a type form like survey form on a react route for an existing site. It should have the following:

• Capture incoming referral url
• This is an objective survey form
• Survey Questions would be provided from backend api via a RESTful Json api with input metadata
• The form is to be rendered based on this json response. For instance, something of the format with Question and objectives alongwith options like radio btn or multiple choice type may be provided in the response metadata itself.
• The form and page should be device agnostic and responsive.

A mock api server is available for testing and building out the form.
