Issue-3: issues when censoring rate is high #9

When pcensor is high, e.g. greater than 0.3 and there are more visit times such as 8, the results tend to produce errors or negative sample size. The higher measurement error, the more likely this issue arises.

Need to update document to define pcensor, the definition is different from pmiss. The value can not be taken arbitrary.
