padgnneco may be a good choice, I've attached the source code (modified to
be compiled with MSVC6, which should be easy to be converted into VS.net).

this plugin works well with both keyboard and joysticks.

the .rc file is in borland format so I just compiled it with Borland C++
Builder and included the compiled .res file in project.

padssspsx is also a good plugin, however PADstartPoll()/PADpoll() interface is 
broken with pcsx-rr which rendered this plugin 
unusable.
