Estonian prescription also has an extension for reason for change. FHIR MedicationRequest has a reason for status change, but the status might not change. T6.2 call 2024-09-27:
When using "reason for change", is it obvious what the change is from? Is it necessary to (sometimes) pinpoint which previous (version of) prescription is the one this change refers to? Is it always the previous version of the same prescription instance?
I think when a new prescription is created, you would not have status change reason. You would have cancelling reason for the old one. If content in the prescription is being changed, it would be for the changes that appeared in this version. And I suppose it's up to the business logic to decide if it stays there forever or until the next change. In Estonia, we only keep the latest change reason on the prescription (earlier versions would be available anyway if history tracing is needed).
HDR guideline has A.2.8.2.2 Reason for change with description "Reason for change of medication".
