Duplicate detection for phone number and address #6

It is highly unlikely that 2 patients share the same phone number but yet live in different addresses. Should add duplicate detection for this.

Thank you for the report. We do want to allow for situations where a patient inputs his/her next of kin contact.

Reason for disagreement: Not particularly satisfied with developer's response. If the next of kin stays with the patient (maybe a child, ward), then their address should be the same and they can have the same contact number. However, if the next of kin stays at a different address, then the contact number should also be different since it doesn't make sense for someone to not live with you but still share your contact number.
