Add VistaDb provider code to source

Code of working VistaDB provider is attached. It will not (probably) be
possible to distribute any support files for VistaDb, so this code can be
included with the source, but not made an active project file. If someone
wants to use it they will have to include in the subsonic project, add the
references to VistaDb and build a custom version of subsonic.

I don't think it's necessary to build. Merely include it for those that want to 
use
it. The VistaDB users that want to use it will understand the licensing problem 
and
perhaps urge VistaDB to changed it's policies.
