Update readme to show the new app name (HedgeDoc) #54

The transition was not made in the readme. Also, I added some information about the app (name) history, as it's confusing for most people

@lapineige I think it's better to not change the Title of the app but only warn about CodiMD beeing renamed HedgeDoc and the fact that this package will not be maintained any more.

I agree. And we should point to the new package. I can't revert the PR (probably because I did 2 other commits afterwards, that I can't revert). Can you do it for me ?

I reverted to where it was. I propose to wait for the HedgeDoc repo to be added in the catalog before adding a warning to this on.
