Accounts approval is now always required #145

Per the second-to-last paragraph of https://docs.gotosocial.org/en/latest/admin/signups/ , accounts approval is always required. We may remove the manifest question and setting.

According to their matrix room we can expect the registration stuff in 0.16.0 in about two weeks. Then we'll probably know whether this setting still makes sense or approval will be needed per default.
