10 years ago I took the source from https://github.com/framasoft/OpenSondage/ there are several forks. Maybe certain versions are better concerning translations. You could fork this repo, and modify source code. I already have modified it (there was "framasoft" references hardcoded).
I think this app is great! I would be happy to help provide English translations, but I'm not totally sure how to do so. Plus, it looks like there are some places in the code where there is only a French option (for example, the "print" button can only show "imprimer" because it's hard-coded). Let me know what is the best way I could contribute.
