This is not a bug, there are 3 relevant background cvars:
i_pauseinbackground (default true) - if true it will pause *everything* when GZDoom loses focus - including sound
i_soundinbackground (default false) - if true and i_pauseinbackground is false then allows sound in background
vid_activeinbackground (default false) - if true and i_pauseinbackground is false then allows video frames to render in background

So... then the bug is that the In-game option did absolutely nothing. This also apparently happens on the Windows version too, as I just had a livestream where I SHOWED it on, but that had absolutely no effect on whether it played sound in background or not.
Now, is this going to be re-opened, or should I be creating a new issue for the new bug?

This might be a post-4.8.2 fix but it was still fixed.

A clear and concise description of what the bug is.
When I go to another application, I do not hear sound or music from GZDoom at all, even though I have turned "Sound in Background" on. This issue does not appear to happen on a MacBook Air from 2015, however, and that might be due to multiple factors, like being on MacOS 10.14.6 instead of 12.5.1, and not being an ARM machine.
