Fix docs #103

There was a merge bug in the README and a broken include in the Makefile

Hey @markulf, this PR still fails.

I think it's worth taking a deep dive at this point. It was my intention to fix this issue after the tech debt of yesterdays PR's were done. So now seems a reasonable time to fix the issues.
