Fix url too long to be displayed issue #81

On Ledger's screen: Use "Version" instead of "Version Format" Use "URL" instead of "Url" The "Name" and "URL" screens on my Nano S didn't show the full name and URL and the fields were not "split" into two screens >(e.g. Name (1/2), Name (2/2), URL (1/2), URL (2/2)).
