Groundwork in anticipation of the M extension (#251).

Introduces virtual registers and virtual instruction sequences
Consolidates instruction, memory, and bytecode traces
Example implementation for MULH

Remaining TODOs:

Implement remaining virtual instructions/sequences cc @mw2000
Make a test macro for virtual instruction sequences, similar to jolt_instruction_test!
Add virtual PC and related constraints to R1CS
Update tracer and toolchain to use RV32IM cc @ncitron
