Create an Athlete Registration Form for a Race

@jsiarto Can you take a look at this? It's hosted here and meets the following criteria:

[x] Choose a sport and race type for your form (running/marathon, sailing/regatta, etc)
[x] Create a valid HTML file using Bootstrap 4 CSS
[x] The form must capture the following: Name, Address (Street, City, State, ZIP), Age, Male/Female, t-shirt size
[x] You must use at least one of each form field type: text, select and radio or checkbox
[x] The form should have a heading, a brief description and be centered on the page
[x] The form should have a submit button (but it doesn't have to actually submit, just show the button)
