Translate IRC channel references to plain text channel names #28

I heard just now that Slack channel references don't translate well outside of the bridge, they end up as ugly opaque identifiers, which can cause confusion. Any chance the bridge could translate them better?

Example:

[snarfed] hacknorris moving to <#C1PA11USK|>

Related to #13 maybe?
