subheading for definition of "start" and "subsection" #46

Two relations are defined under the heading "OPDS Catalog Relations", but it 
seems these should be in their own subsection, like Sorting Relations, 
Relations for Previously Acquired Content, etc.  Maybe this new section should 
be called "OPDS Hierarchical Relations".

We didn't feel that this added structure was required.
