Add docs on error types and output format #22 #30

Fixes #22

  1. Adds sphinx/reST documentation on license scan error classes and the output formats for scancode-results-analyzer.
  2. Fix README, INSTALL, AUTHOR files.
  3. Removes proposal PDF as the docs add necessary and updated information on results analysis.
  4. Adds test scripts for CI/CD and RTD requirements file.
