docs(readme): improve migration note #113

Making the note about migration more visible by splitting it into its own section instead of having it in the section "Why was it renamed commit-and-tag-version?"

Also added content based on a couple of comments starting here: https://github.com/conventional-changelog/standard-version/issues/919#issuecomment-1785678873
