The form should include:

First Name
Last Name
Contact Email
Organization
- Name
- Description
- Goals within Accessibility
- Size of Organization
- Size of Website (?)
- Level of knowledge of digital accessibility within the organization
Website (URL)
Social Media
Evaluation Level selection
- What level of evaluation is the organization potentially interested in?
