WIP: Final fixes before submission to the Denali Commission

Here's what this does

• Adds a micro version of UA's non-discrimination statement to every web page. Fixes #95
• Changes UAF non-discrimination statement to UA's in the PDF version
• Combine PDF pages 2 and 3
• Removes reference to an ACEP report number, which this report won't have because of #105 and #108
• CRediT fixes for Gwen (in progress)
• adds a "Word" rendering profile which did not quite work as expected, so it isn't rendered automatically like the PDF and web-book
