Ensure uniqueness of MINC file names generated by dcm2mnc #556

When the pipeline generates the MINC files, it relies on the default MINC file naming scheme to name the output files. We should make sure that this naming scheme guarantees that the names of the files will be unique for a given MRI pipeline run. Also, no matter what naming scheme is chosen, we should detect if name collisions arise and report this error.
