ENH: Updated to use subject hierarchy 2.0

• No need to make sure SH is enabled, because it is now always enabled
• Use SH item IDs (vtkIdType) instead of node pointers
• Removed utility function to get SH item for data node, as it is a one-liner
• Annotation hierarchies are now supported in SH (need to double check CIP_LesionModel::removeNthNodule and createRulersNode)

Subject hierarchy 2.0 will be integrated to Slicer core soon. In that case SlicerCIP will need to be updated. I did the migration, but could not test.

Please test the related parts when the new SH is in the core. Thank you!

Integration into Slicer core is done, so this branch can now be tested. I may have made mistakes as I could not test it, so please let me know if there is anything to fix! Thanks!
