MIDI implementation for Windows #76

Time for some MIDI action. I think I could activate the old MIDI WIN32 implementation but it will not work on any other platform then.

The code is still in my old snakeoil engine.

Should still work I guess.

It wasn't actually straight forward to port it. Now it even works when the device is plugged off and on and even if the device is newly connected if the program runs. But it still is not finished yet. The midi messages still do not go into any device.
