Add option to exclude a list of games/processes from DX Hooking #52

Some games work fine with DX Hooking, but others crash at startup because of 
Prismatik.

It would be useful to have an option (exclusion list) where we could add the 
games that crash so we dont have to turn off DX Hooking manually each time we 
want to play those games...
