Adapt REDCap data importer to handle repeated instruments with a (unique) field-name duplication #23

NCER-PD instance of Ada absolutely needs this fix since each instrument (medication) is represented by an extra visit row. If not filtered properly this might result in multiple (sparse) rows per each person-visit.

Btw. this is more a REDCap bug than anything else...
