Add cmi5.xml template file #8

Add a cmi5.xml template file so the grunt-replace task can set appropriate values from course.json and store in the root of the course.

Dependent upon merge of https://github.com/adaptlearning/adapt_framework/tree/prep-for-xapi, but the work on the extension is done.
