The noise from the power supply is not rejected very well by this pedal.

Solutions include:

LDO
LC or RC filter at power supply connection to filter out noise more than 1 Hz.
Use linearly regulated power supply designed for audio.

Added holes to pcb to connect an RC low pass or LC low pass filter. Can optionally wire it to not use the filter.

Edit: I removed the holes in the pcb design for the RC low pass since it did not work when using the pcb.
