Implement Scholarship Application Form on Apply Page

The current website lacks a dedicated page for students to apply for scholarships. This is essential for streamlining the application process and collecting relevant user details in a structured manner. Without this form, applicants are forced to rely on manual submissions, causing delays and potential mismanagement of documents.

Describe the solution you'd like.

The new form will allow applicants to submit their personal information, education level, scholarship category, and a short essay about why they deserve the scholarship. Additionally, the form will include a file upload feature for submitting required documents in PDF or DOC format. This will ensure that all necessary details and supporting materials are submitted in a single, user-friendly flow.

Additionally, the code is already prepared and ready for implementation, I kindly request to assign me to this issue

Describe alternatives you've considered.

No viable alternatives were considered, as building a custom form provides better control over the user experience and data handling. Other methods, such as downloadable forms or third-party services, would not offer the same level of customization or efficiency.

Additional context.

This form is part of the new apply.html page and will help applicants easily submit their scholarship applications online. See the attached screenshot for the form's layout and structure. This feature aims to improve the overall application management process and user experience.
