Fix broken headings in Markdown files #1039

GitHub changed the way Markdown headings are parsed, so this change fixes it.

@andyjohnson note that these changes are required in addition to #1034. Looks like the markdown broke in multiple ways.

Working down the list of issues, lots of help (very much appreciated!) but there were a lot of conflicts. As the number of edits is small, could you re-submit to the latest merge? I apologize for not being more on this markdown rendering issue and my latency causing some of this mess in merging.

It looks like @bryant1410 was making fixes like this across Github, so I'm not sure if he'll be back to fix up this PR. I'll try and make some time to sort the conflict.
