This is a very difficult one to reproduce in any reliable way except that it happens constantly to me since CS5. The issue is that when you have many pages with many states in a page (say 15 pages, each page has between 3 and 8 states), and you add a new state and change it's name, sometimes the state name once you press "enter" doesn't take however when you switch to another page you see that the state name you entered is now the name of a state on another page. Worse, the rest of the states are now renamed state 1, state 2, etc.
I normally only have the states panel visible but it seems to happen less when the Pages panel is also visible. I navigate between pages using Page UP/Page Down keyboard shortcuts.
My current theory is that Fireworks gets confused as to what page it's actually on when the pages panel is hidden and you rapidly switch pages using keyboard shortcuts, so when you change the name of a state it applies that to the wrong page and messes up everything else.
I've also seen whole pages adopt the wrong state names on name change of a state in another page.
