ProxyFix: "do not use in non-proxy setup" #98

i wonder why it is here; the documentation says not to use it in non-proxy setup http://werkzeug.pocoo.org/docs/0.11/contrib/fixers/

I believe this was intended due to there being (at one point) the Elastic Load Balancer -> Docker container (which inside had nginx running).
