It doesn’t seem like a QuickLook bug. Could this be a bug with Windows?

To restore the default icon, right-click and select "Restore Default Icon". This issue is likely related to Windows, not QuickLook. If you're unsure where to report a Windows bug, try contacting Microsoft support.
