Refactor/performance #989

Changes:

Refactor repeat items so that it doesn't use state. (Tried to do for repeat groups but it's too unstable so that didn't go through)
Fixed Bug 4 in https://github.com/aehrc/smart-forms/issues/971. Turns out the string and text component is doing a trim() before updating the questionnaire.

Will not be publishing changes into @aehrc/smart-forms-renderer, will leave it in the Smart Forms app first to get some manual testing.
