Slack form needs error handling #24

If form is blank, or missing field, needs to alert error. @NullOverride please fix by Thursday Jul 26th.

For now, I will comment out the modal
Will use the Google form for Slack sign-up (temporarily)

I'm removing the slack integration, we'll just let Google Forms handle input validation and data collection.
