Pardot Form Block rendering as an Eloqua form in Preview mode for VTNA site(s)

Description
When I was attempting to add the Pardot Form block from the PartsASIST.com site to a VolvoTrucks.us page in edit mode, the form block can be added in the word doc, but when I click preview, the form looks like the Eloqua form. We would need the existing Pardot Form Block added to the Sidekick library so that other content editors can use it for both Mack Trucks and Volvo Trucks sites.

Go to a page you would like to add a form to
Navigate to the sidekick library in edit mode
View the options for form blocks

Seeing form blocks for Eloqua forms. We are no longer using Eloqua.
