i18n support in code code set manager #4

Currently i18n support not implemented.   Doctrine currently doesn't
support composite keys well - and the i18n behaviour does not work. 
Roadmap for doctrine has this outlined as a 2.0 feature
(http://trac.phpdoctrine.org/wiki/Doctrine2.0)

Would like to use native doctrine support.
