Allow authentication for unknown MAC addresses #90

This patch will allow NULL entries for UserName in radcheck, radreply, and radusergroup and change the RADIUS queries so that any MAC address is matched if there is an entry with UserName being NULL.

[x] Change RADIUS queries
[x] Change database definitions
[x] Add tests
