HTML Header/Footer Templates No Longer Available? #352

What steps will reproduce the problem?
1. :VimwikiAll2HTML

What is the expected output? What do you see instead?
In the past (couldn't recall which version), generated htmls contained 
customized headers/footers, if 'html_header' and 'html_footer' were specified 
in g:vimwiki_list in .vimrc.

What version of the product are you using? On what operating system?
2.0.1. Ubuntu 10.04 LTS.

Yes, I have forgot to add that to changelog.
