on the licensed access page we could change the image on the form and use the UGRC logo. Probably also want to remove the Title stuff.

For the Popular Disco Resources the raster app link description could say "Download aerial photography and digital elevation models"

I've updated the text in the form to reference UGRC and done other cleanup. The header image has been updated as well.

The titles in the screenshot above are only visible in the edit version of the form. The live version does not display a title if those are left blank.
