DHHS - HII to DHHS AGOL #20

Benefit Hypothesis

Fulfill the last piece of this project and transfer responsibility of hosting the data and app to the DHHS AGOL.

Acceptance Criteria

The data and app are moved to or rebuild in the DHHS AGOL. The link to the app within Health Promotion documents is eventually changed to reflect the new url.

FYI, over the last 6 months the HII app has averaged 1.25 hits per day.
