Support region aware lookup. #18

The code itself is largely inspired by https://github.com/SavchenkoValeriy/emacs-powerthesaurus/blob/93036d3b111925ebc34f747ff846cb0b8669b92e/powerthesaurus.el#L42

This amendment also supports #16
