Issue template that links to the relevant repositories + a note about generating release notes.
A twitter. Eveything needs social media.
An MVP which is just the slack bot (using dialogueflow). Just messaging with "I would like to create an incident" or "incident please" or so to generate a new slack room with some saved note.

It should be possible to use these post mortems as a mechanism to identify patterns such as scarcity, break silos, ...looking for feedback loops.

Maturity model:

immediate resolution
mitigation and regression identification
signalling and production model verification
Architectural verification (i.e. did the system break in a predictable way)
constraint identification and feedback identification (scarcity, silos => identity integrator)
